EVALUATION FAQ


IS YOUR AGENCY LICENSED ?
Yes we are licensed as a DUI Service Provider. All Instructors and Counselors providing services thru RDS are fully credentialed as well.

IS MY INFORMATION SECURE ?
Your information is completely private and secure. The software we use, uses built-in, always-on security measures to protect user privacy and access to data and computers.

WHAT ARE THE BENEFITS OF INTERNET EVALUATIONS ?

  • It's simple and convenient: the entire process can be completed from the comfort of your home, office, or from your attorney's office.
  • It's confidential: no parking in front of a treatment center or sitting in the waiting room of a counselor's office.
  • It's private: the entire evaluation, as well as payment of fees, is conducted over SSL security encrypted internet lines.
  • It’s fast and accessible: the interview takes about 45 to 60 minutes, with no waiting lists for appointments. Services are accessible by anyone able to access a computer or use a phone.
  • It’s less influenced by personal attributes: any person, regardless of race, religion, gender, age, disability, national ancestry, sexual orientation, or economic condition can obtain a bias-free evaluation over the internet or telephone.

I HAVE SPECIAL FORMS, WHAT DO I DO WITH THESE?
Please let us know asap if the your State has specific forms that need to be completed by the agency who prepares your evaluation. Completion of such a form is not part of the evaluation and are subjec to additional charges which can range from $25 - $80, depending on the complexity. Forms can be faxed to 773-822-0793

ARE THERE ANY ADDITIONAL FEES I NEED TO BE AWARE OF?
Additional Fees That May Apply
Depending on your individual situation, the following additional fees may apply:

Some States require additional forms to be completed by the agency providing the services. Please note: We do not have access to forms required by States other than Illinois. If your state requires an additional form to be completed you must obtain it yourself and provide it to our agency.

•Special State forms start at $35 - $85 depending on complexity and the number of pages.( This includes progress reports and letters)
•Notarization Fee is $25
•Express Shipping $35 (overnight via UPS or FEDX)
•Rush Evaluation Fee - if you have a court date we can do a same day evaluation.
•Missed Appointments $60 (if cancelled within 24 hours of appointment)
•Extension Fee - if you do not finish your classes in the required time frame, you are allowed an extension. See Extension Here.


WHAT INFORMATION WILL I NEED FOR AN EVALUATION?
Notice of Summary Suspensions, or
Officers Sworn Report, or
any legal document that contains data of your arrest including
the BAC or Refusal
Police report
Drivers Abstract (Driving History for court purposes)
Copy of Photo ID or Drivers License
Print Out of All Previous Offenses (if any)
BAC or Lab Work Results (if not refused)

HOW DO I GET A COPY OF MY DRIVING HISTORY?
You would need to contact the court and ask for a court purpose driving report.

I HAVE BEEN GIVEN A DEADLINE FOR COMPLETION OF THE EVALUATION / ASSESSMENT WHAT DO I NEED TO DO?
If you have been given a deadline to have either the evaluation, assessment completed by, you must make us aware of this deadline prior to signing up with us so that we can advise if we can meet this deadline. Regular assessments / evaluations are done a first come first serve basis and can be bumped by Priority and Rush assessments / evaluations. There is an additional fee for Priority and Rush as indicated by prices.

WHAT DOES IT COST?
The cost of the evaluation and the written Evaluation Report varies between $200 and $475 depending on the type of evaluation needed. Fees are payable by Visa, Master Card, Discover, American Express, Debit Card, PayPal, cashier's check, or money order.

Assessment prices include:
- Correspondence with a Certified Substance Abuse Counselor
- Assessment Results and recommendations via email your Counselor
- One signed email copy of completed report
- When paying by cashier's check or money order: Payment must be accompanied by a valid e-mail address where we will contact you with further instructions.

ARE THE EVALUATIONS ACCEPTED BY THE COURTS AND DRIVER'S LICENSE DIVISIONS?
Evaluations completed by our Certified Substance Abuse Counselors have currently been accepted by the courts and motor vehicle divisions in most states. Should anyone question the validity of our evaluation, we will work with the court or driver's license agency on your behalf. If they continue to challenge the evaluation, we will refund your fee.* (see refund policy) d(Check Your State)

CAN YOU REFER ME TO A DUI PROVIDER OR TREATMENT CENTER?
Currently we offer online DUI Education Classes only, iIf you require outpatient or inpatient treatment at a Treatment Center, we can refer you to a Treatment Provider that meets your needs.

CAN I USE YOUR DUI ASSESSMENTS WITH LOCAL DUI CLASS PROVIDERS?
Most DUI Class providers usually prefer to do their own assessments or from an designated agency. If it is your plan to take your classes locally, you will need to ask the local DUI Class Provider if they will accept an out of state assessment.

HOW DO I GET MY EVALUATION REPORT?
The original signed Evaluation Report and Referral Reports will be emailed to you from your counselor. It is your responsibility to make copies and forward the original reports to the court, your probation officer, your drivers license division, attorney, or others who require them. Under Federal Confidentiality rules 42CFR, Part 2, we are not allowed to disclose any information about you to any party without your express consent; but you may provide your report to anyone.

I AM TOLD I NEED TO DO A DRUG SCREENING, DO YOU DO THESE?
No, these you must have done locally and are not part of the evaluation. If you are being requested to do one of these in addition to your evaluation you will need to submit these results separately to the requiring authority..
